HOW TO ACCESS YOUR
If you would like to obtain a copy of your medical records, images or test results, we are here to help. To ensure privacy, we require that all request forms be signed and dated by the patient or the patient’s legally authorized representative, parent or guardian. Your medical records are available to you online through the My Baptist Connect patient portal or as hard copies.
ENROLL IN “MY BAPTIST CONNECT” FOR Online Medical Records
As a patient of Baptist MD Anderson Cancer Center, you can view your medical records at any time online through our patient portal, My Baptist Connect.
The My Baptist Connect website is a safe and secure way for you to stay connected to your personal health information online. You can access your personal medical information or send a question or comment to your provider at any time. My Baptist Connect makes it easy to access your medical records, including:
- your diagnosis
- test results
- your vital signs
- your blood counts
- current medication list
- any medications you are allergic to
- clinical summaries
You can also enter your own health and medical information into My Baptist Connect and you’ll have everything in one place for quick online access!
My Baptist Connect
To enroll, with your consent, we’ll email you a secure personal information number, or PIN. Once you receive your PIN, use it to complete your registration at MyBaptistConnect.com — the secure link will be included in your email invitation. If you cannot find your PIN or need assistance, call 1.844.622.0622.
HOW TO OBTAIN HARD-COPY Printed Medical Records
In order to receive a physical copy of your medical records, images or test results, you will need to complete one or both of the following forms and send or fax it to the Baptist MD Anderson Patient Access Services representative on the first floor of our campus.
Authorization to release or obtain medical information form
You will need to complete and submit this form to the Health Information Management (HIM) office in order to receive your medical records such as:
- your entire medical record (no films)
- your medical history & physical
- your consult records
- your discharge summaries
- emergency department records
- laboratory results
- radiology reports and imaging films
- pathology reports
- cardiovascular reports
- operative reports
